Hospital Indemnity Insurance is a voluntary benefit that supplements your medical coverage. It pays a daily benefit if you or a covered family member are admitted to a hospital, critical care unit, or rehabilitation facility. The benefit amount depends on the plan. Payments are made directly to you and can be used however you see fit.

| Wellness Benefit | If you or your spouse/domestic partner complete a health screening, you will each receive $50, payable once a year from Voya. Each child who completes a health screening will receive $25, with an annual maximum of $100 for all children. Learn more. | |||||||||||||||||||||
| Filing a Claim | Once your coverage goes into effect, you can file a claim for covered illness/condition on the Voya website. | |||||||||||||||||||||
